What is in-home care?

In-home care is support and care provided within your own home, or out in the community, to assist you in maintaining your independence and continue living in your own home. 

It includes services such as:

  •           Domestic assistance

  •           Personal care

  •           Nursing services

  •           Social support

  •           Transport services

How do I access home care services?

You will need an aged care assessment to access home care services.  To arrange an assessment please call My Aged Care on 1800 200 422, alternatively you can request an assessment on their website https://www.myagedcare.gov.au/referral. Once you have your assessment you will need to select your provider and apply for care services.

What if I am not sure what I need and for how long?

The benefit of in-home and community care is that is it very flexible and designed to be responsive to the individual’s needs. At the commencement of your services, your needs will be assessed to work out what services you might need and whether support is required on a temporary or ongoing basis. A plan will then be developed in consultation with you and this plan will be reviewed and adapted as needed. Our friendly staff are always available and happy to discuss your needs on (03) 9942 7106 or 0403 731 035 or complete an enquiry form.

What happens if my health needs change?

If your health needs change, we will work with you to ensure your care plan changes to meet these needs.

How do you choose your caregivers?

One of the reasons we are so successful is our ability to match clients and caregivers. After our initial meeting and assessment with you or your loved one, our Case Manager will have an excellent idea of the level of care needed and the caregiver that would be a good fit.  However, if at any time you wish to change caregivers, for any reason, we can happily arrange this for you.

Do I need a referral?

No, you do not need a referral to access Empower Homecare’s services. You can contact us directly to arrange support for yourself or someone who needs some extra care.

To access services through a government funded package, we require that you have your aged care assessment and details of the package for which you have been deemed eligible.

Do I have to sign a contract?

No, we do not require a long-term commitment to our services. We want you to use us only if you are 100% satisfied.
